About

About CID.

Independent in judgement. Accountable to the evidence.

The Clinical Investigation Division is the dedicated clinical investigation and patient safety practice of Princeton Lee Partners.

Understanding what happened is essential, but it is not enough. We also establish why it happened, how the conditions for harm developed, and what must change to reduce the risk of recurrence.

To reveal what matters, establish what happened, and contribute to safer clinical care.

Why we exist

The signals are often there.

Adverse clinical events are not always sudden, isolated or unforeseeable.

  • Deterioration may develop over hours or days.
  • Abnormal observations may accumulate without their significance being recognised.
  • Information may be lost at handover.
  • Decisions may remain unchanged as new evidence emerges.
  • And when harm occurs, the record may not fully explain how events unfolded.

These are not necessarily failures of individuals alone. They may reflect weaknesses in clinical processes, communication, decision-making, organisational behaviour or governance.

Understanding them requires examining the clinical episode as it occurred, using the evidence available at the time, and following that evidence wherever it leads.

That is why CID exists.

How we work

Identify. Investigate. Prevent.

Three interconnected functions. Each can operate independently; together, they form a disciplined approach to understanding clinical harm and reducing its recurrence.

01

Identify

Recognise clinical warning signals, emerging risks and patterns of potential harm.

Explore Identify
02

Investigate

Establish what happened and why, through independent, structured examination of evidence, decisions and systems.

Explore Investigate
03

Prevent

Translate investigative understanding into stronger clinical processes, safer decisions and effective risk controls.

Explore Prevent

Our methods are complementary capabilities within a common investigative approach, drawing on clinical risk assessment, systems science, behavioural analysis and enterprise governance.

  • CASEClinical Assessment & Systems-Level Evaluation
  • FIRST-PASSPreliminary Clinical Safety Review
  • PASS+Patient Adaptive Safety Surveillance
  • CORAClinical Observation & Risk Analyser

Independence & ethos

Neither fear nor favour.

Our work may involve patients who have suffered harm, clinicians whose decisions are being examined, and organisations facing difficult questions. Each deserves a process grounded in evidence, fairness and respect.

We do not approach investigations with predetermined conclusions. We assess decisions within the circumstances in which they were made, without hindsight, and distinguish established facts from interpretation and uncertainty.

The commissioning party does not determine the findings.

This applies whether we are engaged by an individual, a healthcare institution, a legal representative or another body.

Our commitments

  1. 01Fidelity to the available evidence
  2. 02Independence of investigative findings
  3. 03Fairness to those whose actions and decisions are examined
  4. 04Transparency about methods, limitations and uncertainty
  5. 05Respect for patient dignity, confidentiality and wellbeing
  6. 06Accountability for the quality and integrity of our work

Our first duty is to patient safety. Our highest allegiance is to the truth.

Our identity

Our methods may be commissioned. Our findings must be earned.

CID is a division of Princeton Lee Partners Pty Ltd, an independent Australian firm experienced in enterprise architecture, complex program assurance, organisational risk, systems analysis and technology governance. CID applies these disciplines to clinical environments.

Our methods can be commissioned and licensed. The CID crest cannot: it appears only on work we have conducted ourselves and stand behind.

Our principles are not for sale.

Where an investigation requires specialist clinical expertise, relevant professional input is obtained and considered. CID is an independent professional investigative practice. It is not a statutory investigative authority, healthcare regulator or substitute for a treating clinical team.
